Which Aflac claim form do I need? (PDFLiner)

The right Aflac claim form depends on two things: what kind of policy you have, and what you are claiming for. Aflac sorts its claim forms by policy type first, so start there.

Key Takeaways

  1. Start with your type of policy: Aflac supplemental, network dental, vision and hearing, or Aflac Group certificate.
  2. Then pick the form for your type of claim; Aflac Group lists its forms by claim type and has separate New York forms.
  3. You can file online 24/7 through MyAflac or send the completed forms with supporting documentation by fax or mail.

Start with your type of policy

Aflac's page for filing a claim by fax or mail asks you to "select your type of policy and follow the instructions." It offers three routes:

  • Aflac Supplemental Policy Claims, for individual Aflac supplemental policies.
  • Aflac Network Dental, Vision and Hearing Claims.
  • Aflac Group Certificate Claims, for coverage through an employer's Aflac Group plan.

Group coverage has its own forms on the Aflac Group claims page.

Then pick the type of claim

On the Aflac Group page, the forms are listed by claim type: wellness benefit, accident, BenExtend, cancer, critical illness, dental, disability, hospital, group life or accidental death and dismemberment rider, and universal life underwritten by Trustmark. Aflac Group lists separate forms for New York, and Aflac Life, Absence and Disability customers (formerly Zurich) file life, disability and absence claims through a separate member portal.

For individual policies, Aflac's claims checklist page has a separate checklist for each claim type, including accident, cancer/specified disease, critical illness, hospital and short-term disability. Aflac notes that a checklist "does not constitute a guarantee of claims payments or act as an all-inclusive list."

PDFLiner's pages on the Aflac initial disability claim form and the continuing disability claim form explain what those forms ask.

File online or on paper

Aflac's claims hub says you can "Submit your claim online 24/7" and manage your account, submit and track claims, and set up direct deposit through MyAflac. To file on paper, Aflac says to pick from its options, "complete your claim forms and send to us with all necessary supporting documentation." Use the fax number or address in the instructions for your policy type; Aflac lists separate claims fax numbers, for example one for Aflac Group and one for Aflac New York.

If you disagree with a decision

For Aflac policy claims, the fax or mail page says you can appeal a claims decision up to a maximum of three times per claim, and that all appeals must be submitted within 180 days of the original claim decision. Different procedures apply to claims through Aflac Life, Absence and Disability Solutions.
